POSITIONS AVAILABLE

Clinical Manager, West Team - Monday - Friday 8 a.m. to 4:30 p.m. - Provide leadership in coordination and implementation of clinical services to patients and families in Prince George's County. Participate in long and short-range planning requiring the formulation of goals, objectives interventions, policies, procedures and standards. Provide clinical direction to staff and evaluate the level of care provided by staff to ensure safe, appropriate and effective care. Participate in hiring, orienting and evaluating staff by joint visits and evaluations. Ensure that all paperwork is accurate and timely to maintain record. Understand and accept the hospice philosophy. Knowledge of Medicare, Medicaid, private insurance and other community programs and resources. Basic computer knowledge and skills. Participate in Administrator On-Call rotation. Minimum of five years' experience in one of the stated disciplines in healthcare field and prior hospice experience required. Minimum of three years of supervisory experience required. Very strong clinical skills. Excellent oral and written communication skills. Demonstrated ability to work effectively on an interdisciplinary team. Demonstrated emotional maturity and ability to cope and work effectively with stress and multiple responsibilities. Ability to function independently with minimal supervision. Demonstrated supervisory and leadership skills.
RN/Case Manager, West Team - Monday - Friday 8 a.m. to 4:30 p.m. - Assumes responsibility and accountability for a group of patients. Renders professional care in accordance with the treatment plan and the nursing process of assessment, diagnosis, planning, implementation and evaluation. Functions as a member on an interdisciplinary team, provides case management and primary nursing care to assigned patients in Prince George's County, addressing physical, psychological and spiritual aspects of care. May participate in weekend, holiday, on-call and evening/night rotations. Current Maryland RN license. Valid driver's license and car insurance. Graduation from an accredited nursing program. Two year's recent nursing experience, preferably in medical/surgical area. Must recognize safety issues to self and others, be able to assess and implement use of lifts or other personnel to lift and move patients of all sizes and weights in a safe manner. Must be able to function well in stressful situations and have good, creative, problem-solving skills. Excellent interpersonal, organizational, written and verbal skills. Ability to handle multiple assignments and projects with efficiency and accuracy. Basic computer skills needed for automated record keeping.
Admissions RN - Provides liaision and admissions visits during four rotating days weekly 8 a.m. to 8:30 p.m. and every third weekend.. Current Maryland license and valid driver's license and car insurance. Graduation from an accredited nursing program with two years recent experience preferably in the medical/surgical area. Possess a thorough knowledge of nursing principles. Demonstrates ability to provide quality patient care. Demonstrates ability to assess patient's needs clinical, mental and spiritual. Basic computer and keyboarding skills necessary for automated medical recordkeeping. Willingness and ability to pass a drug screen. Previous hospice experience a definite plus. Four rotating days weekly 8 a.m. to 8:30 p.m. and every third weekend.
Lead Inpatient Nurse Practioner Palliative Medicine - Primary member of the palliative care consulting service responsible for both patient care and program development. In addition to exceptional clinical skills, candidates will have education and administrative experience that allows them to participate in the marketing and operations of the consult service. The patient population consists of hospitalized patients who are living through the late stages of cancer, chronic illness or dementing and debilitating diseases. The NP will see patients in the hospital and in the inpatient hospice setting. The NP will be part of the medical team with other physicians and nurse practioners to meet the needs of this population and will participate in an on-call pool for night and weekend coverage (approximately once every five weeks). Masters in nursing with a specialization in Adult, Geriatric or Family Medicine, Oncology or closely related fields. Active Maryland NP license and state and federal controlled substance registration. Board certification in hospice and palliative medicine preferred. Minimum of five years of experience in treating seriously ill adult patients. Experience in ICU preferred. Demonstrated understanding of and commitment to, the philosophy of hospice and palliative care. Familiarity with hospice, home care and adult social services. Fluent in the ethical and legal issues surrounding end-of-life care and prescribing controlled substances. Excellent interpersonal, organizational, written and verbal communication required. Basic computer skills, including knowledge of word processing and the ability to provide automated recordkeeping on patients. Monday - Friday 8 a.m. to 4:30 p.m. plus on-call rotation.Hospice of the Chesapeake requires background and reference checks for all positions and drug screening for all clinical positions.
