At a time when patients and their families are in need, your contribution will make a difference in providing special care for seriously ill patients and peace of mind to grieving families.
Whether it's medication to reduce pain, care at one of our two facilities or providing a patient care volunteer, your contribution will help patients receive whatever they need for maximum comfort.
HOW TO SUPPORT HOSPICE OF THE CHESAPEAKE
A memorial gift is a meaningful way to honor the memory of someone who has touched your life.
Show someone how much their care or support has meant to you by making a gift in their honor.
Annual gifts can be a wonderful way to celebrate or commemorate the birthday or anniversary of a loved one. Gifts given annually help Hospice of the Chesapeake provide direct care to patients and families, regardless of their ability to pay.

A positive way to ensure Hospice care is available for future generations is to support the Hospice Endowment Program. Endowment gifts are held in trust, generating resources that will provide services and programs in perpetuity for future generations.
PLANNED GIVING, ESTATE DONATIONS
In addition to the benefits of lowering inheritance taxes, donations made to Hospice of the Chesapeake clearly express your wishes to family and friends. There are some key guidelines to remember when making a bequest:
- Contact your attorney for advice about establishing or amending your will to include a donation to hospice.
- Name Hospice of the Chesapeake as the recipient of the donation.
- Include any special instructions to Hospice of the Chesapeake regarding the use of your donation.
- Indicate the dollar amount or the total percentage of your estate for your gift to Hospice of the Chesapeake, as well as naming any specific belongings or real estate holdings to be distributed to the organization.
Individual Retirement Accounts can be taxed up to 70% if passed directly to your heirs. By designating Hospice of the Chesapeake as your beneficiary, your inheritance taxes are greatly reduced. Contact your Retirement Plan Administrator and name Hospice of the Chesapeake as the beneficiary of your donation today.
Estate planning. Donations made through life insurance are a means of benefiting Hospice of the Chesapeake directly. Probate is avoided, passing donations without delay.
By donating stocks and mutual funds to Hospice of the Chesapeake, you avoid paying taxes on capital gains. In addition, any donations of full market value, if held for 12 months, are tax-deductible.

Per our recent telephone conversation, I wish to make an irrevocable gift to Hospice of the Chesapeake of "x" shares of "XYZ" Corporation. Please transfer the stock to Hospice of the Chesapeake via DTC as follows:
