By focusing on symptom management and empowering you with self-care techniques, our dedicated team helps you stay out of the hospital and emergency room—delivering care in the comfort of your home.

If you’ve experienced frequent hospitalizations, are on optimal heart medication doses, and continue to struggle with symptoms, it may be time to explore hospice care. Our program is tailored to help you live more comfortably and manage symptoms effectively.
How Our Program Stands Apart:
To qualify for our program, you must meet hospice eligibility (a serious, life-limiting illness with a life expectancy of six months or less).
Our Advanced Cardiac Care Program offers specialized support, including:
Personalized Care: Frequent contact with a nurse specially trained in managing cardiac symptoms.
- Symptom Control: Medications and protocols that help manage your symptoms so you can continue doing what you love.
- Preventative Care: Monitoring to track changes and prevent your symptoms from worsening.
- Holistic Support: Additional care from Chaplains, Social Workers, Certified Nursing Assistants, and Volunteers to support your emotional and physical well-being.
- Access to Holistic Treatments: Opportunity to explore non-medical interventions that focus on healing and the well-being of the whole person.
