What is GUIDE?
The Guiding an Improved Dementia Experience (GUIDE) initiative is a federal program designed to support individuals diagnosed with any stage of dementia. Our organization has been selected by CMS to offer this program, which aims to:
- Enhance the quality of life for people with dementia
- Alleviate stress on unpaid family caregivers
- Help participants stay at home and in the community longer
Key Features
- Fully covered by Medicare, with no extra cost for those enrolled
- In-depth comprehensive assessment and personalized care planning
- Seamless coordination with existing healthcare providers—you don’t need to switch doctors
- A dedicated care navigator to help connect you and caregivers with local programs and services
- Extensive caregiver support, including training, education, and access to a 24/7 helpline
Who Qualifies?
To enroll in GUIDE, participants must:
- Have a formal diagnosis of mild, moderate, or severe dementia, confirmed by our physician or nurse practitioner
- Be enrolled in Medicare Parts A and B (but not Medicare Advantage or PACE)
- Not currently be in hospice care
- Not reside in a long-term nursing facility
